Modern Elevations is an in-network provider for most insurance plans in Ohio. If you are interested in becoming a patient but do not see your insurance plan listed below, please contact our office for assistance. Please contact your insurance provider directly for any questions regarding your specific insurance plan.
Payment is expected at the time that services are rendered, unless additional arrangements have been made with our office. As a courtesy and a convenience to you, we will accept direct payment from your insurance company. However, you are responsible for any annual deductibles and copayments as required by your insurance company. Additionally, you are responsible for any treatment costs that exceed your annual maximum benefits as determined by your insurance company. The financial relationship is between you and our office. Therefore, you have the final responsibility and obligation for payment of your bill.
For greater flexibility for our patients, our office accepts Care Credit for the payment of out-of-pocket dental expenses. If you are interested in applying for Care Credit, please contact our office for assistance.
